Cataract

There is a normal crystal clear, transparent lens located inside the eye. This crystalline lens gradually starts losing its transparency and become opaque of various grade.
The natural, clear lens becomes non transparent, opaque and the vision becomes dull. Ageing is the main cause for cataract. It may also occur due to injury to eye, chronic intraocular infection and due to prolonged intake of oral or topical steroids.
Cataract is not a fleshy overgrowth on the eye.

Presbyopia

Normal lens is transparent and elastic in nature. Upto 40 years this elasticity is maintained so that there is no defect in near vision. At the age of 40 due to loss in the accommodation of the lens, reading becomes difficult. This is called presbyopia. To correct this defect presbyopic glasses are advised.
Presbyopia will affect myopic, (defective distant vision) hypermetropic (defective near vision) and also cataract patients.
Some people prefer reading without glasses after Cataract surgery. For them Multifocal IOL can be used.

Astigmatism or cylindrical power

Our cornea is more valuable than gold and diamond. It is smooth and even in all 360°. For some people the cornea is sharp in one angle so distraction of light happens in that particular angle. This is called as astigmatism.

Contrast

Our natural lens has the power of differentiating minute details in perception of colors in perceiving vision with highest clarity, etc.
This contract power is very high in 15 years of age. It reduces after 25 years. After cataract formation this contract power reduces considerably.

Causes of Glaucoma

The passages that normally allow fluid within your eye to drain become clogged or blocked. Fluid within your eye then builds up and increases pressure on the optic nerve. The nerve fibers and blood vessels in the optic nerve are easily damaged by this pressure, resulting in vision loss.
An injury, infection or tumor in and around the eye, matured cataract can increase the intraocular pressure.

Corneal Transplant Services

The cornea is the transparent part of the eye that covers the front portion of the eye. It covers the pupil (the opening at the centre of the eye), iris (the coloured part of the eye), and anterior chamber (the fluid-filled inside of the eye). The cornea's main function is to refract, or bend, light.
Corneal blindness is scarring of the cornea caused by a wide variety of infectious and inflammatory diseases, leading to vision loss. A mild to severe scratching or scraping of the cornea (e.g., a corneal abrasion) as well as a wide range of congenital, infectious, and inflammatory diseases can lead to cornea-related vision loss and blindness. Individuals with limited access to protective eyewear (e.g., low-wage agricultural labourers and factory workers), deficient in Vitamin A, or vulnerable to bacterial, fungal, or viral infections, especially young children and older adults, are at higher risk for developing corneal blindness.
Keratoplasty is the corneal transplant procedure in which the diseased patient corneal tissue is excised and replaced with healthy donor cornea. Either full thickness of the cornea or a part of it may be transplanted.

What is cataract?

There is a normal crystal clear, transparent lens located inside the eye. This crystalline lens gradually starts losing its transparency and become opaque of various grade.

Why does cataract occur?

- Commonly due to old age-(Senile cataract).
- Misuse of steroid tablet or steroid eye drops (steroid induced cataract)
- Due to injury with blunt or sharp objects (traumatic cataract)
- Infection to the mother at the time of pregnancy leading on to cataract to the new born (congenital cataract)
- Eye infections caused by bacteria causing tuberculosis, leprosy etc., and other eye infections causing uveitis leads on to a cataract called complicated cataract.
- Other conditions like night blindness, detachment of the retina also can cause complicated cataract.

Are cataracts found only in older people ?

Most cataracts develop slowly over time and affect people over age of 45 . in rare cases ,infants can have congenital cataracts due to infection in mother during pregnancy.

Why does cataract occurs in younger age?

It could be a pre-senile cataract like premature greying of hair .It could be because of borderline nutritional deficiency in younger individuals.

What is the reason for cataract progression /development?

Mainly due to aging ,other factors contribute for cataract progression are any kind of vitamin deficiency or malnutrition, prolonged exposure to sunlight(UV rays ) can initiate a cataract formation and also cause progression of cataract from one stage to another .

Can cataract be treated by applying eye drops or by wearing glasses ?

No ,cataract can be treated only by surgery(replacing the natural lens by IOL ).

What are the symptoms of cataract ?

- Gradual decrease in vision
- Cloudy,fuzzy,foggy or filmy vision
- Haloes,for example seeing shadows around a bright object like vehicle head lights
- Experiencing glare especially in night driving
- No clarity in vision even with new prescribed glasses
- Sometimes experiences double vision

Why do cataract patients experience photophobia /flashes/glare ?

There is hydration of lens fibres in cataract and formation of vacuoles through which when light passes there is diffraction causing photophobia or glare.

How does cataract develop, after any eye injury?

In case of penetrating injury, a sharp object pierces the cornea i.e. the black button of the eye, there is also simultaneous injury to the capsule of lens. This capsule is the protective covering of the normal lens. Once it is injured by a sharp object, the lens material also gets injured. It becomes immediately opaque called Traumatic Cataract.

How cataract is diagnosed ?

Cataract is diagnosed by dilated comprehensive routine eye check .

My doctor says I have a cataract, but he wants to wait a while before removing it. Why?

- Mild cataracts often cause little or no vision problems. Your doctor is probably monitoring your cataract to see if it worsens and more significantly affects your vision or lifestyle before recommending surgery.
- Some cataracts never reach the stage where they need to be removed. But if your cataract worsens and you begin to have trouble seeing clearly for driving and other everyday tasks, it's probably time to consider cataract surgery.

How cataract development relates with diabetes ?

- The normal lens has got lens fibres. These fibres have a tendency to opacify under situations of low blood sugar levels (hypo glycemia ) or high blood sugar levels (hyper glycemia).
- In diabetic patients there is a rapid progression of cataract from immature to mature stage.
- Sometimes there is fluctuation in vision whenever there is fluctuation in the blood sugar levels which causes transient cataract.

When cataract surgery is advised ?

When patient starts to experience cloudy ,foggy ,double vision and glasses no longer provides clear vision ,which lead to difficulty in performing daily activities when the cataract becomes matured ,it’s advisable to undergo cataract surgery .

How is a cataract removed ?

A small incision is made in the front surface of the eye with a scalpel or a laser. A circular hole is then cut in the front of the thin membrane (anterior capsule) that encloses the eye's natural lens. Typically the lens is then broken into smaller pieces with a laser or an ultrasonic device so it can be more easily removed from the eye.
Once the entire lens is removed, it is replaced with a clear implant called an intraocular lens (IOL) to restore vision. In most cases, the eye heals quickly after surgery.
Today, several steps in cataract surgery can be performed with a computer-controlled laser instead of hand-held instruments.

How much does cataract surgery cost?

The cost of cataract surgery varies from one doctor to the next; it also depends on the type of procedure and intraocular lens you and your cataract surgery decide is best for your needs.Most of the cataract surgery is covered by insurance.

What are possible side effects of cataract surgery?

Is there any option so that I can avoid wearing glasses after cataract surgery? Will cataract surgery correct my glasses power ?

Cataract surgery is different from lasik ( surgery done to correct power of eyes ) .during cataract surgery ,your ophthalmologist remove the cataract lens and replace it with artificial IOL Where else in lasik the black part of the eyes ( cornea ) is corrected to achieve clear vision without glasses.
- Presbyopia is a condition most people over the age of 40 experience that results in difficulty seeing up close without the aid of bifocals, trifocals, or reading glasses.
- This age-related condition is believed to be caused by a hardening of the lens inside the eye, making it difficult for the eye muscles to focus.
- This can be corrected during Custom Cataract Surgery using a multifocal or presbyopia-correcting IOL.
- Another type of premium lens available is a Toric (monofocal) IOL that can correct astigmatism.
- Astigmatism is caused by the cornea being shaped more like a football than a basketball, which results in images focusing at different points in the retina. As everyone’s eyes vary, you will need to meet with your ophthalmologist to determine if one of these premium lenses can help reduce or eliminate your dependency on glasses after your cataract surgery.

Is cataract surgery is painfull?

No,anesthesia is given during the surgery so that you don’t experience any kind of pain .

Are you awake during cataract surgery ?

Yes, typically you are awake during cataract surgery. This eliminates risks associated with general anesthesia (being "put to sleep") and enables your cataract surgeon to communicate with you during your procedure.

There is no time limit, it can be done even on the same day, especially for special cases like patients with other medical ailments who cannot be frequently mobilized for surgery.

Does cataract surgery causes dry eyes?

Cataract surgery does not typically cause dry eyes.
Only two very small incisions are made in the cornea during the procedure. These incisions heal quickly after surgery and do not significantly affect how many tears your eyes produce.
Though cataract surgery does not cause dry eyes, a pre-existing dry eye condition can sometimes affect the quality of a cataract surgery outcome.
For this reason, during your eye exam and consultation prior to cataract surgery, your eye doctor or surgeon will evaluate the tear film on your eyes. If it appears you have dry eyes, treatment is given to restore your tear layers prior to surgery.

Does cataract surgery cures floaters ?

Cataract surgery restores vision that has been diminished due to clouding of the eye's crystalline lens, which is located directly behind the pupil.
Because vitreous floaters occur in the posterior part of the eye (the vitreous body is located in the large cavity in the back of the eyeball, between the lens and the retina), removing a cataract and replacing it with a clear intraocular lens (IOL) will not diminish the appearance of vitreous floaters.
In fact, if you have vitreous floaters, the increased clarity of your vision after cataract surgery may make the floaters even a bit more visible.

If you see large spots and floaters, treatment may be possible?

What is glaucoma?

It is a condition that cause damage to the optic nerve, which transmits information from the eye to the brain. It usually (but not always) is associated with high intraocular pressure (IOP). Left untreated, glaucoma can cause blindness.

What are the symptoms of glaucoma?

There are usually no symptoms that you're developing glaucoma until vision loss occurs, which is why it's so important to have regular eye checkup. Your eye doctor can detect and treat high IOP before it progresses to optic nerve damage and vision loss.

How glaucoma is diagnosed?

During the routine eye checkup, the IOP (intraocular pressure) will be measured by tonometry (NCT-non-contact tonometry) and dilated fundus examination will be done. Why can’t I leave the patching until my child is older and I can reason with him/her?

Is it worth me patching at all? A two-hour dose of patching has been recommended, but my child will only wear the patch for half an hour.
Yes, any patching is better than none at all and you may be able to gradually increase the length of time the patch is worn as your child’s vision improves.

Do I have to do the patching in one session each day? What do I do if I forget one day?

The patching can be worn continuously for the prescribed time or it can be split up eg. 2 hours continuously or two 1 hour slots – whichever fits best into your routine.
However, many parents find that splitting it up causes more "fuss" from the child! If you forget to patch one day try to patch for twice the recommended time the next day.

I have noticed the squint sometimes swaps over into the good eye. What should I do?

If the squint swaps into the good eye occasionally this is a good sign for the vision, it means that the vision is becoming more equal in the two eyes.
If the squint just swaps over occasionally keep patching for the recommended time.
If at any point you think that the good eye is turning more than the “bad” one then you should stop the patch and contact your eye clinic for advice.

How many hours a day will the patch have to be worn for?

There is good evidence that 2 hours of patching a day is as effective as 6 hours of patching for moderate cases of amblyopia (vision between 20/40 – 20/80 or 6/12-6/24).
In more severe amblyopia patching for 6 hours per day is usually recommended.

How does an infant get ROP?

Stage 3 ROP exists when these disturbing new blood vessels grow out from the ridge in the retina toward the center of the eye. If this blood vessel growth becomes severe and is accompanied by “plus” disease (enlarged and twisting blood vessels in the back part of the eye), the child may reach the point where treatment of the peripheral retina with laser (or rarely freezing) treatment is performed. When stage 4 or 5 ROP is reached, the retina is detached

Can ROP get better or heal on its own?

Yes – in fact most infants do. It is called " regression " of the disease, and occurs when the retinal blood vessels grow to cover and nourish the retina.

Can ROP lead to blindness?

Yes. Eyes which progress to stage 4 or 5 can go blind. This may happen even when a child is properly screened and is properly treated in a timely fashion.

What treatments are available for ROP?

Currently, the standard of care treatment for ROP severe enough to require it is destroying the retina without blood vessels with laser. On occasion, freeze-treatment is used to accomplish the same goal. By destroying the peripheral retina the goal is to save central vision so that the infant can have as normal vision as possible. Other experimental treatments are being studied as well – most notably a drug called Avastin (bevacizumab).
Once the retina detaches (Stage 4 or 5), there are surgical therapies which may be employed to salvage vision.

How are at-risk infants followed?

They are examined either at the bedside or by digital pictures, generally on a weekly basis.

Is my child out of risk in terms of ROP once she/he is discharged??

Usually not. Infants at risk for ROP are followed until 52 weeks after conception. Since most infants are discharged from neonatal intensive care well before that age, in general some exams are still necessary in the weeks following hospital discharge to ensure the best visual outcome for your child.

How important are follow-up visits for ROP?

Failing to keep follow-up appointments puts your child at risk of blindness.

When are infants with ROP treated?

Most infants requiring treatment for ROP are treated at about 35-36 weeks post-conceptional age (typical range 32-40 weeks).

What is the treatment procedure?

The baby’s eyes are dilated, a lid speculum is used to hold the lids open, and the peripheral retina is treated with the laser. Treatment can be done either under general anesthesia, or sedation with local anesthesia.

Is treatment for ROP painful?

With anesthesia it would be a painless procedure. There is no pain after the treatment is completed.

What is the treatment success rate?

On the order of 85% of eyes requiring laser are effectively treated.

What will my child’s vision be after treatment?

It depends on the stage and severity of ROP. The less scar tissue present at the time of treatment, the greater the potential for visual development. Eyes treated at stage 2 or 3 may see as well as 20/40 or better. Eyes at early stage 4 often see as well as 20/80 - 20/200. About half of eyes at more severe stages of stage 4 see better than 20/2000 (ambulatory vision). Ambulatory vision is defined as being able to see objects and move around a room without stumbling or bumping into obstacles. Surgery for stage 5 ROP is often done to preserve vision in the range of ambulatory vision to light perception.

What is the impact of the laser on a child’s field of vision?

Peripheral vision (“side vision”) is usually reduced by laser treatment. Some side vision is sacrificed in the interest of preserving central vision – what we all use to read, drive, etc

What if treatment fails?

When stage 4 or 5 ROP is reached, the retina is detached and other therapies can be performed. One such therapy is scleral buckling, which involves encircling the eyeball with a silicone band to try and reduce the pulling on the retina. The most commonly performed surgery is vitrectomy (removal of the gel-like substance called the vitreous that fills the back of the eye). Sometimes the removal of the lens as well is required if the retina is touching the back surface of the lens, which would make it impossible to enter the eye.

What happens once the ROP regresses?

ROP is a lifelong disease, whether it resolves after treatment or gets better (regresses) on its own. Formerly premature children and adults require ongoing care for near sightedness (myopia), amblyopia (“lazy eye”), retinal detachment, and early cataract.

Does my child have any activity limitations because of ROP?

Usually not. Most children with regressed ROP live normal lives. On occasion, children with more advanced stages of ROP may have limitations with regard to sports and other activities.

How often should a child needs follow up??

Its depends on the stage of ROP,usually 4-6months follow up is advised initially. As the condition stabilize yearly follow up is compulsory.

Mahathma Eye Hospital has conducted so many programmes and camps from 1997 till 2013 with the holistic approach to quality eye care to everyone. After 2013, in association with Mahathma Eye Vision and Research Institute (MEVRI) trust. Though this trust is created and constituted by Mahathma Eye Hospital on 25th February 2013 as a public trust for the purpose of promoting a public charitable medical institution for treatment of all kinds of illness and education, training, research related to treatment of eye, we are proud to say that we were already doing huge social activities from 1997 onwards. They are as follows:

In School Eye Screening programme, we have screened 19,68,588 school students from 5,485 schools in which 99,116 were diagnosed with refractive errors and 12,399 spectacles were issued free of cost.
In College Eye Screening programme, 1,39,307 college students have been screened and benefitted.
In Integrated Child Development Scheme Staff (ICDS) Eye Care programme, 15,347 employees were benefitted.
Through Noon-Meal Workers Eye Care programme, 3,698 employees were screened for eye defects and benefitted.
In Rural door to door Eye Care Services, 21,374 poor people were benefitted.
In Diabetic Retinopathy, Screening programme, 39,070 patients were benefitted.
In Pensioners Eye Care programme, 16,908 patients were screened and benefitted.
In Senior Citizen’s Eye Care programme, 719 patients have undergone eye screening and benefitted.
In Women Self Help Group Eye Care programme, 2,128 women participated and screened for eye disorders.
Factory/Industries Workers Eye Care programme, 10,659 employees were benefitted.
In Apartment Dwellers Eye Care programme, 8,816 people were screened and benefitted.
In DSWO-Special Children Eye Care programme, 1,264 children were screened and benefitted.
In Eye Awareness Exhibition Visitors Eye Care programme, 25,828 visitors’ eyes were screened and benefitted.
In Door-step Survey Eye Care programme, 16,579 people have been screened and benefitted. They have performed about 829 free cataract surgeries supported by Mahathma Eye Hospital.
In 2018, we have done a project, “Eradicating Needless Blindness in Children” in Tiruverambur block, Trichy district. In this project, we have screened 23,906 number of students from 109 schools and given treatments for Refractive errors, Squint, Nystagmus, Mentally Challenged Children, Ptosis and others. 250 students were provided with free glasses.
In 2019, we have done a School Screening programme in Seva Sangam Girls Higher Secondary School, Trichy district. In this programme, 1609 students were screened and free glasses were provided to 75 students.

ACHIEVEMENTS:

Mahathma Eye Hospital has conducted so many programmes and camps from 1997 till 2013 with the holistic approach to quality eye care to everyone. After 2013, in association with Mahathma Eye Vision and Research Institute (MEVRI) trust. Though this trust is created and constituted by Mahathma Eye Hospital on 25th February 2013 as a public trust for the purpose of promoting a public charitable medical institution for treatment of all kinds of illness and education, training, research related to treatment of eye, we are proud to say that we were already doing huge social activities from 1997 onwards. They are as follows:

In School Eye Screening programme, we have screened 19,68,588 school students from 5,485 schools in which 99,116 were diagnosed with refractive errors and 12,399 spectacles were issued free of cost.
In College Eye Screening programme, 1,39,307 college students have been screened and benefitted.
In Integrated Child Development Scheme Staff (ICDS) Eye Care programme, 15,347 employees were benefitted.
Through Noon-Meal Workers Eye Care programme, 3,698 employees were screened for eye defects and benefitted.
In Rural door to door Eye Care Services, 21,374 poor people were benefitted.
In Diabetic Retinopathy, Screening programme, 39,070 patients were benefitted.
In Pensioners Eye Care programme, 16,908 patients were screened and benefitted.
In Senior Citizen’s Eye Care programme, 719 patients have undergone eye screening and benefitted.
In Women Self Help Group Eye Care programme, 2,128 women participated and screened for eye disorders.
Factory/Industries Workers Eye Care programme, 10,659 employees were benefitted.
In Apartment Dwellers Eye Care programme, 8,816 people were screened and benefitted.
In DSWO-Special Children Eye Care programme, 1,264 children were screened and benefitted.
In Eye Awareness Exhibition Visitors Eye Care programme, 25,828 visitors’ eyes were screened and benefitted.
In Door-step Survey Eye Care programme, 16,579 people have been screened and benefitted. They have performed about 829 free cataract surgeries supported by Mahathma Eye Hospital.
In 2018, we have done a project, “Eradicating Needless Blindness in Children” in Tiruverambur block, Trichy district. In this project, we have screened 23,906 number of students from 109 schools and given treatments for Refractive errors, Squint, Nystagmus, Mentally Challenged Children, Ptosis and others. 250 students were provided with free glasses.
In 2019, we have done a School Screening programme in Seva Sangam Girls Higher Secondary School, Trichy district. In this programme, 1609 students were screened and free glasses were provided to 75 students.

DNB Program Overview

Overview

DNB (Ophthalmology) is a residential post-graduate programme in ophthalmology encompassing both academic and clinical aspects of the discipline.
We accentuate morals, excellence and humanity while training residents to become good ophthalmologists with responsibility oriented towards the society. Our students are trained to manage patients in wide spectrum of eye diseases. A good hands on training in cataract surgery is the hallmark of the PG training program. Throughout the residency period they will be guided on their thesis projects also.
Each resident will get to do around 1000 surgeries during their PG period. Our residents are also trained in sub specialities according to their thesis topic. This will encourage them to do fellowship in that speciality and improve their skills. We have fully equipped library with all latest books and National and International journals. Regular academic classes and journal club are conducted in a systematic manner. Residents are encouraged to regularly participate in CME programs as well as conferences of TNOA and AIOS.
Classes will be conducted on a regular basis with case presentations at least once a week, didactic lectures thrice a week and Journal Club presentations every week. Faculties of National and International fame from India and Outside India will be engaged for the benefit of postgraduates. They will undergo written examination on every month following the discussion on specific topics. NBE conducts the annual appraisal for all the candidates (Theory & Practical) as per NBE Guidelines.
As Mahathma Eye Hospital is fully equipped with diagnostic equipments, PGs here will be given free hand in using those equipments and sharpen their diagnostic skills as well.
Each candidate is given a logbook for recording interesting clinical cases they’ve seen, discussed, presented, surgeries and minor procedures that were performed. Proper maintenance of log books is a must for all candidates. Appraisal of log book and attendance is carried out every 3 months.

We are very keen in imparting excellent skill transfer in OPD management, Diagnostics and more than anything in Operation Theatre. We whole-heartedly encourage the MBBS completed candidates to join our 3 years primary DNB and the DO completed candidates to apply for our 2 years post DO curriculum.

SURGERY: Hands on training in Cataract Surgery. The OR is fully equipped with modern technology Microscopes and high resolution Camaras. Wet Lab surgical practice is a unique feature.
DIAGNOSTICS: All the speciality OPDs are fully euipped with premium equipments. Work freedom, utilising these equipments will be imparted for all the PGs.
RESEARCH: Research & Publications department that provides support in publishing the articles in both National & International Journals.

The recent World Diabetic Week, spanning from 14th Nov to 20th Nov 2023, witnessed a series of impactful initiatives aimed at elevating awareness and support for diabetes care..

A standout event on November 18th and 19th was the expansive mega camp held across six vision centers, notably including Mahathma Eye Hospital Private Limited, Trichy, Tamilnadu. Among the 152 patients assessed during the camp, 32 cases of Proliferative Diabetic Retinopathy (PDR) were identified, emphasizing the urgent requirement for early detection and specialized care. Intensified efforts, led by Dr. Anugraha Prasanna and Dr. Prasanna Venkatesh Ramesh, ensured swift referrals and access to specialized treatment for these individuals.

On November 14th, the radio programs served as a cornerstone in disseminating critical information and lifestyle advice for diabetes care to a wide audience. Dr. Anugraha Prasanna, our esteemed Chief Vitreoretinal Surgeon, led an enlightening live Q&A session broadcasted on All India Radio Trichy. Notably, during this session, we formally announced the upcoming two-day free diabetic camp.
The Diabetic Week's social media campaign, featuring captivating flyers, highlighted vital aspects like early detection and lifestyle changes for effective diabetic care, engaging diverse audiences with essential information.

Media coverage by prominent TV channels amplified the significance of the November 18th press meet at our hospital. It united stakeholders, healthcare professionals, and leaders, highlighting challenges, successes, and community support for those affected by diabetes.

These initiatives, carefully organized during Diabetic Week, showcase our commitment to a healthier community. As we celebrate these achievements, we persist in advocating proactive measures, educating widely, and fostering support for those battling diabetes.
